I would get your iron levels checked, your thyroid checked, get your hormone levels checked including testosterone. If you are dieting or anorexic, stop. Be careful not to take too much vitamin A. I would also increase your protein intake and get tested for autoimune diseases like lupus as patchy hair loss is one symptom.
First, go to your family doctor and get some blood work done just to rule out anything serious... then... if not... it's likely Alopecia.... bummer. Hereditary in my family. Thankfully I have a full head of hair.
I've heard from friends that caffein shampoo works, but don't know for sure. They buy it on ebay from some guy in Germany...
There's a great website, wigsalon.com that has ';pull through'; hair enhancers. You wear it interspersed with your own hair, that way it doesn't feel l ike a wig. Looks natural too.

My hair was falling out and I was almost bald and it was hyperthyroidism.
Definitely get a blood test for all your thyroid levels.
